A no-show is not simply a missed appointment. It is an empty chair, an idle assistant, and a slot that could have gone to someone else. In a three-dentist clinic even two no-shows a day add up to a serious number by the end of the month.
The good news is that most no-shows are predictable, and they are solved by process rather than by conversations.
Why patients do not come
Look at the real reasons, not "the patient was irresponsible":
- They forgot. The appointment was booked three weeks ago and no reminder was sent.
- They could not cancel. The only way was to phone during working hours, and the patient was at work.
- They did not realise it was confirmed. Booked by phone, nothing in writing.
- They are afraid. Especially before root canal treatment and extractions.
The first three are solved by reminders and easy cancellation. The fourth needs a conversation, but it is visible in advance from the procedure type.
What works
A reminder 24 hours ahead. This is the main tool. Not a week ahead — they will forget again by the appointment. Not two hours ahead — too late to refill the slot.
WhatsApp instead of a call. In Armenia and most of the CIS, almost everyone reads WhatsApp and few answer an unknown number. The message is seen immediately, can be answered whenever convenient, and stays in the thread as confirmation.
The ability to reply. A reminder that cannot be answered is half useless. If a patient can write "I can't make it" the day before, the slot can still be filled.
A waiting list. When you have a list of people willing to come sooner, a freed slot closes with one call. Without a list it simply disappears.
Confirmation at the moment of booking. A short message with date, time and address at booking removes an entire class of misunderstanding.
What not to do
No-show fees look logical but usually cost private dentistry more than they return: they are awkward to administer, they damage relationships, and they barely affect the people who genuinely forgot.
Repeated reminders irritate. One the day before and one at booking is enough.
How to measure
Track one number: no-show rate = no-shows ÷ scheduled appointments. Monthly, and separately per dentist.
Without this number it is impossible to know whether a change helped. With it, the effect of reminders is visible within a month.
If the rate is higher for one dentist, the cause is usually not their patients but how appointments are booked: too far ahead, or without confirmation.
Order of implementation
- Start measuring the no-show rate — before changing anything
- Turn on confirmation at booking
- Turn on the 24-hour WhatsApp reminder
- Build a waiting list for the next two weeks
- Compare the number after a month
